School personnel will administer minor first aid. Serious injuries will receive first aid by the office, or by the emergency services of the Eagle Mountain Fire Department. Parents are required to sign an authorization for emergency care, which identifies the physician and hospital preferred in the event that the parents cannot be reached. Parents should keep this information current by notifying the school of any changes.

No child with a fever above 100 degrees will be allowed to remain in class. Any rashes or eye infections may require the child to go home when there is a question of contagion. A child may return to school when the symptoms disappear and there is no longer a possibility of contagion.

The school office will conduct or assist with the processing of visual and hearing screening of students each year. Parents will be notified if the screening indicates further examination might be needed.

In accordance with the state laws, immunization records must be on file at school and kept current. The State Health Department audits Trinity Baptist Temple Academy immunization records annually to assure compliance with state regulations. Parents must respond promptly to inquiries from the school relating to these records to maintain student enrollment at Trinity Baptist Temple Academy.

Communicable Diseases

A pupil who has an infectious or contagious disease shall be temporarily removed from school until the danger of transmitting the infection or disease to other students has passed. Students may re-enter only by presenting evidence to the principal or supervisor that the significant risk has abated. Students should remain at home until they have been free of fever for 24-hours.

Chicken Pox
2-3 weeks, commonly 13-17 days
Exclude until all blisters are crusted and dry and child is free of fever.
Diphtheria
2-5 days, maybe longer
Exclude case and/or close contacts until released by physician.
Infectious Hepatitis
15-20 days, commonly 25 days
Exclude until one week after onset of illness.
Impetigo
4-10 days
Exclude unless under treatment by a physician.
Lice
Eggs hatch in one week
Exclude until child is treated with a medicated shampoo for lice. Re-treatment necessary in 7-10 days for newly hatched eggs. Research indicate that total removal of nits is essential.
Measles, Rubeola
7-14 days
Exclude until released by physician or Health Department. (Serological proof of disease is required for Vaccination exemption.)
Measles, Rubella
14-21 days
Exclude until released by physician or Health Department.
Meningitis
2-10 days, commonly 3-4 days
Exclude until statement from physician that the person is non-infectious. Exclude household contacts for 72 hours observation period.
Mononucleosis
2-6 weeks
Exclude until released by physician.
Mumps
12-26 days
Exclude until released by physician.
Pink Eye
24-72 hours
Exclude until recovery, or Physician's statement that the person is non-infectious and is under treatment.
Ringworm
(skin or scalp)
4-14 days
May attend school provided person is under treatment of a physician.
Scabies
several days or weeks
Exclude until under treatment and no open lesions are observed.
Streptococcal Infections
(scarlet fever, scarletina)
1-3 days
Exclude until released by physician, or until 24 hours after antibiotic treatment is begun.
Tuberculosis
from infection to primary lesions about 4-6 weeks
Exclude until released by physician.
Typhoid Fever
range 1-3 weeks
average 2 weeks
Exclude case, carrier, and/or household until released by physician.
Whooping Cough
(Pertussis)
commonly 7 days usually within 10, no more than 21 days.
Exclude until released by physician, and completion of 5 days of antibiotic therapy.

Immunizations

The following immunizations are required for admission or continued attendance:

DTP and/or TD

18 Months - 4 years: 4 doses
5 - 16 years: 4 or more doses with at least 1 dose after the 4th birthday.
7 - 11 years: 3 or more doses with at least 1 dose after the 4th birthday.

"booster" doses of tetanus/diphtheria (TD) vaccine are required every 10 years after the dose obtained on or after the 4th birthday. Oral or injected (OPV/IPV).

POLIO

18 months - 4 years: 3 doses
5 - 11 years: 3 or more doses with at least 1 dose after the 4th birthday.

MEASLES (RUBEOLA)

18 Months - 4 years: 1 dose after first birthday.
5 - 11 years: 1 dose after 1st birthday.
Second dose required for those born on or after 9/2/91.
12+ years: 2 doses - 1 dose after 1st birthday and 1 dose on or before the 12th birthday.

MUMPS

18 Months and Older - 1 dose after 1st birthday.

RUBELLA (GERMAN MEASLES)

18 Months or Older - 1 dose on or after 1st birthday.

HEMPOPHILUS INFLUENZA (HibCV) --type of conjugate vaccine

15 Months - 4 years: Minimum 3 Hib prior to 15 months or 1 Hib on or after 15 months.
5+ years: none required

Medication

Prescription medications for all students MUST be sent to the school in original containers. The school office must keep medication needed by students. A student may NOT keep medication in his pocket, desk, etc.

A handwritten permission slip (copy in back of handbook) from home shall be accepted for one day only. It must include the date, child's name, name of medication, time, amount of dose, and parent's signature.

A note from the physician is required if a student must keep a medication on his/her person (doctor only), and must be turned into the office.
